All grant recipients are required to provide the foundation with a final report no later than twelve months from the date of grant notification. Reports should detail how monies were expended and what the project achieved in measurable terms. Additional interim reports may be required during the grant term dependent upon the type of project or program funded. All reports to the Foundation should include your agency name, the name and phone number of the person submitting the report and the name of the project funded. (See "Final Report" form below for details) Immediately inform us of any changes in address, phone number or personnel. If there are unanticipated circumstances that would affect the the project, this also must be brought to our attention.
We encourage you to share your insights into what worked, what did not and changes you might make to improve your progress. Your candor and open feedback not only inform us of your progress, but provide us with valuable learning experiences that enable us to grow, improve our grantmaking, and help other non-profit organizations at the same time. |
